Student-Athlete On-Line form

This questionaire is to be filled out by a student-athlete enrolled at The Sage Colleges. This form is for The Sage Colleges' Director of Athletic Communications use only.

All the information listed below will be used/released (with the exception of phone numbers/addresses and email addresses) by the athletic communications office for publicity purposes. Please be as specific as possible and provide honest answers. We need to know your hometown newspaper(s) so we may include them on an e-mail list for our hometown press release program. Please list your full name the way you want it to appear in all materials, including rosters, produced by the athletic communications office.
