Please take the time to fill out this new feature of an on-line student-athlete questionaire!
This questionaire is to be filled out by a student-athlete enrolled at The Sage Colleges. This form is for The Sage Colleges Sports Information use only.
All the information listed below will be used/released (with the
exception of phone numbers/addresses and email addresses) by the
sports information office for publicity purposes. Please be as
specific as possible and provide honest answers. We need to know
your hometown newspaper(s) so we may include them on an e-mail list
for our hometown press release program. Please list your name the
way you want it to appear in all materials, including rosters,
produced by the sports information office.
